Instructions

Complete this form online using a standard web browser (e.g., Edge, Chrome) without any unnecessary plugins or extensions. Some browser plugins/extensions may prevent the online form from working correctly or prevent you from uploading your transcript. If you encounter any problems, send an email to FIHSmuseum@gmail.com with the subject line, “2024 Dr. Weis Scholarship.”

Please provide all information requested in the application instructions. Two references not from an immediate family member must be provided. You will need to upload a digital copy of your most recent high school transcript (or report card if transcript is not available) showing your cumulative GPA in pdf, jpg, or png format. For most schools, you should be able to obtain a copy of your transcript in pdf format. The Peninsula School District uses a company called “Parchment” (parchment.com) to provide transcripts. Its website (https://www.psd401.net/services-and-resources/request-and-report/transcript-request) provides information on how to use the service.

When your application form is successfully submitted, you will receive an email from our website (foxislandwamuseum@gmail.com) with a copy of the information you provided. This email will be sent to the address you provide on the application form. Please check your email spam folder before contacting us if you believe your form was successfully submitted but you do not receive the email. Your application submittal date and time are recorded by the museum website.

High Schools Attended

List all high schools (or equivalent) with the dates you attended (from/to) and overall GPA for each school. Upload a digital copy of your most recent transcript and/or report card below, preferably using the same information submitted to your college or vocational training program.

References

Please provide information for two adults over 21 years of age who are not related to you and are willing to discuss your character, activities, and interest in further education. You should let these people know that they will be contacted by a scholarship committee member by phone or email. Please include their full name, email address, and phone number as well as their relationship to you (e.g., family friend, neighbor, teacher, employer). These people should know you well (e.g., character, interests, activities).

Essay Question

In an essay of 500 or fewer words, please discuss your interest in one or more of the following areas, and how your experiences, classes, activities, and/or paid or volunteer work connect to them: history, education, community service, communication, science, business, and technology. In addition, please address how the college or vocational/technical training program you have chosen suits your passion or enthusiasm for your interests and goals.
